Our Process
Step-by-Step Process for NDIS Participants Accessing Our Services
1. Initial Inquiry
The participant or their carer contacts us via phone, email, or online form to learn about our programs and discuss their needs.
2. Needs Assessment & Consultation
We schedule a meeting (in-person, online, or by phone) to understand the participant’s goals, support requirements, and preferences.
3. Review of NDIS Plan
We review the participant’s NDIS plan to identify funded supports that match our programs. Guidance is provided if plan adjustments or reviews are needed.
4. Service Proposal & Agreement
A tailored service plan is created, outlining supports, schedule, goals, and costs. The participant reviews and signs a service agreement.
5. Program Enrollment
The participant is enrolled in appropriate programs (e.g., respite, in-home support, life skills, Passion Pathways) based on their goals and plan.
6. Trial Shift & Buddy Support (What Makes Us Different)
Before fully matching a carer, we offer a trial shift to ensure the participant and carer are a great fit.
Each new carer also completes a buddy shift alongside an experienced team member before working independently with the participant.
7. Service Delivery
Supports are provided by qualified staff in line with the service plan, either in-home, at our facility, or in the community. Progress is monitored regularly.
8. Ongoing Review & Feedback
Regular check-ins ensure the services are meeting the participant’s needs. Adjustments are made as goals evolve or NDIS plan changes.
9. Reporting & Communication
Progress notes, incident reports, and updates are documented and shared with the participant, their support network, and the NDIS as required.
10. Continuous Support & Growth
We continue to provide flexible, person-centered supports, encouraging skill development, community participation, and achievement of personal goals.
